Credit Card Form: Allows families to authorize one-time or repeating fees, including tuition payments, to be charged to designated credit card.
Lunch Request Form: To be completed by parent to designate whether the child will purchase a lunch at the Callier Center or bring a lunch from home.
Meat Substitution Request: Allows parent to authorize substitution of cheese for identified meat(s) when family preferences or medical considerations apply.
Medical Information Form: To be completed by the child’s pediatrician, required before child starts attending.
Medication Form: Required for administration of any medication by school personnel (note: all medications, including over the counter types, must be accompanied by authorization of pediatrician).
Notification of Dietary Needs: Substitution of food items in school-provided meals requires this form to be completed and signed by child’s health care provider and updated annually.
School Lunch Vacation Request Form: Parents may complete this form to temporarily suspend charges for lunch.
