Policies
All children should be picked up by 10:30 p.m. Overtime charges are added at the rate of $5 for each 10-minute time slot or portion thereof. This charge is due and payable when you arrive to pick up your child. If a child is left chronically after 10:30 p.m., the child will be terminated from the program.
Neither the DIS nor UT Dallas is responsible for lost or stolen articles. Please put names on all belongings and clothing.
The child care teacher is available daily for consultation. Problems or occurrences affecting a child will be brought to the attention of the parents. A conference may be scheduled by appointment.
Parents will be liable for damages caused by their child, i.e. broken windows, holes knocked in walls, defacing of walls, etc.
A child may be terminated from the program for continual disruptive or abusive behavior and language.
All persons picking up children will be required to sign a sign-out sheet.
The person who brings the child to the program must stay until the child has been checked in by the teacher. A child must not be left to find his own way into the building and classroom.
The children are not allowed to buy soda or candy from the machines until the parent comes to pick them up.
Children are encouraged to leave personal toys at home.
Enrollment
Children between the ages of 4 and 11 years will be admitted. Immunizations against childhood diseases must be up-to-date in accordance with Texas state law.
Enrollment forms must be completed and submitted to DIS personnel. (The Parent's Class Schedule form must be updated each semester.)
Child care services may be used only during the hours that a parent attends classes. Services may not be used while working in the library or while attending other University functions.
The DIS is non-discriminatory in policy regarding admission, programs, and activities.

Hours and Holidays
Child care hours are from 4:30-10:30 p.m., Monday - Thursday. Parents must sign their children both in and out each time they attend.
The DIS/UTD child care service is open from the first day of class each semester to the last day of finals for that semester. Child care is closed on official, UT Dallas holidays.
Notification of a child's absence should be made by 4 p.m. of the day prior to the absence, if known, or as soon as possible the day of.

Payments and Fees
The cost per day for child care is $6 per child.
For example, if there are four possible class days in the month, that month's fee will be $24; costs for 2 children will be $48.
Payment of fees is due on the first day of each month. A late fee of $10 will be charged for payments that are three (3) days delinquent. Failure to keep current with monthly payments will result in termination from the program.
Absences
The FULL monthly fee is due regardless of any absences during the month. Whether a child is present or not, the monthly fees are payable in advance and late charges are applicable.
Refunds
Fees will not be refunded based on illness or other unforeseen reasons; unused fees may not be applied to the following month.

Health
The DIS/UTD Child Care Program can only accept well children. We are depending on you to help us maintain this policy. All children will have an informal health check on arrival to school. Symptoms that are cause for keeping your child home are: coughing, red eyes, flushed skin, earache, chills, headache, sneezing, skin eruptions, sore throat, fever and cold.
Should a child become ill while in our care, we will contact you immediately. You must pick up your child or arrange for an approved person to pick him/her up immediately upon notification (no exceptions). We will provide isolated care until your child is picked up.
Complete medical information must be provided on the Health Information form and any additional instructions included.
If medication is necessary, please contact the child care lead teacher to sign written permission and instructions for use. No medication will be administered without written information.
Minor first-aid will be administered for scrapes, cuts, bumps, etc.
If an injured child should need immediate medical emergency treatment, the child care teacher or other designated individual will call 911 and contact you immediately.

Release of Children*
A list of individuals to whom a child may be released shall be kept on file. It shall be updated as appropriate.
Individuals picking up a child must be adults unless otherwise authorized. A sign out sheet must be signed by the individual picking up a child before the child is released. Only individuals designated on the enrollment form may have a child released to them unless otherwise indicated.
Telephone calls regarding the release of a child shall be confirmed by returning all calls to the appropriate individuals. Release of a child to individuals not already designated shall be confirmed by direct phone contact with the child’s parent, guardian, or other appropriate individual.
All employees must require proof of identification from individuals attempting to pick up a child. Any unknown individual must be required to show proof of identification. Proof of identification shall be a Texas driver’s license or a Department of Public Safety Identification. If picture verification is not available, the individual must provide three items for identification; other identification will be accepted as appropriate. The ID card number must be documented and kept for 24 hours.
Parents’ student ID or other numbers will be required to verify identification over the phone. A release record for each day shall be kept on file for no less than one month. All employees must refer to current release records before releasing a child. Employees must refer individuals to the Director if there are questions concerning the release of a child.
*If there is a custody dispute, parents must provide a copy of the court’s custody arrangement.
