General Problems/Issues Form

This form is for reports of general Arts & Humanities-controlled facilities issues. These facilities include Jonsson Performance Hall (JO 2.604), University Theatre, Visual Arts Building, the Media Room (AS 1.105). Issues to report may include media equipment, lights, furniture, doors, floors, as well as any other general problem. The reports will be immediately forwarded to the person(s) responsible for correcting the issue.
