Documentation

Disability Services adheres to the documentations standards established by the Educational Testing Service (ETS).  For the office to make critical decisions regarding the needs of a student with disabilities while the student is on campus, it is essential that we adhere to these standards.  Documentation must be from a qualified professional and must be current.  Qualified professionals can include physicians, psychiatrists, licensed psychologists, or educational diagnosticians.  Specific requirements can be viewed by following the links on the left hand side of this page.

Documentation must address your specific disability, including the diagnosis, ensuing limitations or impairments resulting from the disability, and any recommendations for accommodations.  Please note that the recommendations from qualified professionals will be considered when the Disability Services Coordinator reviews a student’s information; however, an independent decision regarding accommodations will be made.  Also, note that you must provide documentation on a timely basis for the office to serve you.
