Authorization for Professional Services

The Authorization for Professional Services (AFPS) form is used to authorize payment for professional services in amounts greater than $600 and for payments of any amount made to non-residents. Use the Individual Service Provider Documentation Form For Payments Less Than $600 that are made to U.S. citizens or residents.

The Employee/Independent Contractor Classification Checklist is a form that needs to be completed and processed with the AFPS form.
