What is Evidence of Insurability?

 

Evidence of Insurability (EOI) is the record of a person's past and current health status.  It is used by insurance companies to verify whether a person meets the definition of good health. A physical is not required before completing the application(s). If you are making a coverage change that requires EOI, all EOI applications must be electronically submitted through UT My Benefits or postmarked by August 15, 2011.

My UT Benefits Online Evidence of Insurability

In the upcoming Annual Enrollment period from July 15, 2011 through July 31, 2011, you may be required to complete an EOI form if you are making changes to your life insurance coverage, disability coverage, and long term care coverage. EOI is no longer required for Medical coverage.

If EOI is required for you or your eligible dependents, you will be required to fill out your EOI application electronically on My UT Benefits.  After providing answers to all questions on the EOI online application, you will electronically sign your EOI form upon submission and you can print a copy for your records.  Do not to send the application to Human Resources as these applications are sent directly to the vendor performing the EOI evaluations.

Insurance Choices That Require Evidence of Insurability:

  • Electing or increasing the level of employee voluntary term life insurance.
  • Electing or increasing voluntary term life insurance for a spouse.
  • Electing voluntary term life for retiree spouse. 
  • Electing or increasing the retiree voluntary term life insurance.
  • Electing Long Term Care.

The application review may take six to eight weeks to be completed. Should additional information be required, the processing time may be extended. You will be notified if additional information is needed to complete the review. The actual approval or denial of your application is made by the Medical Underwriting Department at Dearborn National.  UT Dallas Human Resources Management does not make decisions concerning EOI applications.

As soon as the application review is complete, you will be notified by the insurance company via letter of either the effective date of coverage or the declination of the application. At that time, Employee Benefits Services will process your change to reflect the proper coverage and payroll deduction.
