Adding a Child
Children under the age of 25 are eligible to enroll in our insurance programs. However, you must enroll them within 31 days from the date of the event (i.e. date of birth; date of adoption; date of legal guardianship). Dependents, including newborn babies, are not automatically covered under your policy.
What Do You Need to Do?
To enroll your child you must contact your Benefits Rep and schedule an appointment. You need a copy of your child’s birth certificate or, in the case of an adoption or legal guardianship, legal court documents.
Required Forms
- Insurance Enrollment/Change Authorization Form
- Dependent Information Form
- Change In Status Form
- Birth certificate, or legal guardianship papers, and for stepchildren, a copy of the marriage certificate
- Special Dependent Form (for stepchildren, adoption and legal guardianship) - contact HRM for the form
When Does Coverage Begin?
As long as you enroll your dependent within the 31-day time frame, coverage is effective the first of the month following the date of the event.
