Section 25: Verbal, Written or Automated Notices of Appointment Reminders

Purpose: To allow the patient the ability to be exclude from receiving verbal, written or automated telephone notices of appointment reminders from UTD.

Policy: Each patient will be asked if he/she wishes to be excluded from verbal, written or automated notice of appointment reminders by UTD, see Authorization Form.

Procedure: Patients will indicate on the Authorization Form if they wish to be contacted by verbal, written or automated notices of appointment reminders.
