Directory Update

At times, an employee’s office location, office phone, department, admin unit, etc. must be updated. To do this, the admin unit completes a Personnel Action Form and submits the form to Human Resources Management.

Required Form:

Personnel Action Form (PAF)

What the admin unit needs to do:

  • Complete Section A on the PAF.
  • Fill in UTD-ID and employee name.
  • Fill in only the information that you want changed/corrected.
  • Complete Section E contact information; sign the form and send to HR10.
