Buying Health Insurance Online

• Log onto www.uhcsr.com.
• Under College Students select Find My School's Plan.
• Type UTD, click search. Select The University of Texas at Dallas for school.
• Select Enroll Now.
• For the Demographic Information page
• Fill out your date of birth.
• If you have a spouse that you would like to insure on this plan click on YES.
• Enter the number of children you would like to insure.
• Enter your current zip code and select Texas for state.
• Click on continue.
• Under the appropriate student type, select the coverage period that you are purchasing: annual, semester, or quarter.
• On the next screen, the Optional Major Medical Benefit is listed. If you choose to purchase this optional coverage, it will increase your medical coverage from $100,000 to $500,000 per each illness or injury. This option costs $592.00/annually for the 2009-2010 policy year.
• Create an online account with a user ID and password. Once done, fill out the application form with your personal information and address.
• You will then be given a choice to pay by credit card or electronic bank draft. Fill out the numbers accordingly.
• After completing the required information and selecting your payment information, the insurance company will notify you by email when they have processed your application and payment

Buying Health Insurance by Mail

• Download 2009 Enrollment Form.
• Mail the completed form with your payment to:

  Dallas, TX 75380-9026

Please Note: Eligible students who purchase this policy may also insure their dependents.  Dependent eligibility expires concurrently with that of the insured student.  Non-student dependents are not eligible for services provided by the UT Dallas Student Health Center.
