Insurance Policy Details

  • The policy covers a lifetime maximum of $100,000 for each illness or injury, repatriation and evacuation benefits.
  • There is a $300 accumulative annual deductible per insured person for the academic year, August 20, 2009 through August 20, 2010.
  •  When using the nationwide United Healthcare Options PPO network, covered services are paid at 80%.  After the deductible has been met, the insured's co-insurance amount is 20% of all costs, excluding prescriptions.
  • Out-of-network covered expenses are paid at 60% of Usual and Customary charges.  The insured's portion is 40% plus 100% billed above Usual and Customary charges. It benefits you financially to stay within the assigned network.

A Primary Care Physician (PCP) can treat most illnesses.  If the PCP decides that a specialist needs to be consulted, the PCP can issue a referral to you.  In order for the insurance company to cover a consultation fee with a specialist, you must have a referral from your PCP or the Student Health Center (SHC).  Without the referral, coverage for a specialist consultation fee will be denied by the insurance company. To obtain a referral from the SHC, call (972) 883-2747 and schedule an appointment.

Prescription Benefits

The maximum prescription benefit is $1300.00 per academic year.  You must purchase the prescription in full from a pharmacy of your choice.  You may then file a claim for reimbursement. To get the most of out of your prescription benefit, ask your doctor if generic drugs are available.

Repatriation and Evacuation

In the event of the insured's death, repatriation benefits will be paid for preparing and transporting the deceased to his/her home country.  Evacuation benefits will be paid if the insured person is hospitalized for at least five consecutive days and recommended and approved by the attending physician for the evacuation of the insured to his/her home country.
