Fall 2009 waiver period has ended. Insured students will begin receiving their insurance card by mail shortly.

UT System Medical and Repatriation/Evacuation Insurance Policy Requirements
- Medical benefits of at least $50,000 per accident or illness
- Repatriation of remains benefits of at least $7,500
- Medical evacuation benefits of at least $10,000
- Deductible not to exceed $500 per accident or illness
- Policy plan meets Federal Solvency Guidelines: ..more
- Policy coverage dates cover all of mandatory coverage period
- Policy pays benefits while you are in the United States

1) Fill out the SHI Waiver Form, attach with supporting documentation showing proof that this alternate insurance meets UT System requirements.

- Your waiver will be processed within 2 business days. If you are covered by insurance that does not include repatriation/evacuation coverage, you will be assessed a fee for enrollment in an approved repatriation/evacuation plan.
- You will receive an email to your UT Dallas account when your waiver has been processed.
Please Note: This two-day processing applies throughout the waiver period. Students who submit SHI waivers on the day of or the day before any university payment deadline will have to pay the SHI fee and then request a reimbursement from the Bursar’s Office after the waiver is processed.
