The policy dates and premium rates are preset.  If you purchase a policy after the semester starts, the premium will not be pro-rated by Student Insurance.

The Basics

The Board of Regents of the U.T. System require international students to have approved health insurance and repatriation/evacuation coverage while enrolled at U. T. Dallas and U. T. System component institutions.  Health insurance is optional for domestic students.

Each semester, when international students enroll for classes, they will automatically be assessed the Student Health Insurance fee.  This fee fluctuates each semester. The fee purchases the Student Health Insurance policy which offers $100,000 (U.S.) in medical and hospitalization benefits for each covered illness and injury.  Included in the policy are repatriation and evacuation benefits.

The policy covers a lifetime maximum of $100,000 for each illness or injury. There is a $300 accumulative annual deductible per insured person for the academic year, August 21, 2007 through August 20, 2008.  When using the nationwide UnitedHealthcare Options PPO network, covered services are paid at 80%.  After the deductible has been met, the insured's co-insurance amount is 20% of all costs, excluding prescriptions.

Out-of-network covered expenses are paid at 60% of Usual and Customary charges.  The insured's portion is 40% plus 100% billed above Usual and Customary charges. It benefits you financially to stay within the assigned network.

You may choose a primary care physician (PCP) from the UnitedHealthcare Options PPO Network.  PCP's may only be chosen from lists of Family or General Practitioners.  (This policy considers Internists to be specialists.)  To search for a provider, visit www.myuhc.com.

A PCP can treat most illnesses.  If the PCP decides that a specialist needs to be consulted, the PCP can issue a referral to you.  In order for the insurance company to cover a consultation fee with a specialist, you must have a referral from your PCP or the Student Health Center (SHC).  Without the referral, coverage for a specialist consultation fee will be denied by the insurance company. To obtain a referral from the SHC, call (972) 883-2747 and schedule an appointment. The SHC is located in room 1.606 of the Student Union. You must be a currently enrolled UT Dallas student with a valid Comet Card to be seen at the Student Health Center.

Prescription Benefits

The maximum prescription benefit is $1000.00 per academic year.  You must purchase the prescription in full from a pharmacy of your choice.  You must then file a claim for reimbursement.  Your reimbursement check will be subject to a $15 co-pay for brand name drugs and a $10 co-pay for generic drugs.  To get the most of out of your prescription benefit, ask your doctor if generic drugs are available.

Exclusions and Limitations

Inform yourself about the Exclusions and Limitations of this policy at United Healthcare Student Resources.  Some exclusions include assistant surgeon fees, acne, dental, vision, hearing, routine physical exams including well-woman exams, and all preventative care. Some limitations include no more than one surgical procedure will be covered when multiple procedures are performed through the same incision or in immediate succession, and outpatient physical therapy except for a condition that required surgery or a hospital confinement, etc. Allergy medicines are covered and included in the $1,000 maximum under prescription drugs. Benefits for sexually transmitted diseases are covered up to $500 maximum per policy year, etc.   

Repatriation and Evacuation

In the event of the insured's death, repatriation benefits will be paid for preparing and transporting the deceased to his/her home country.  Evacuation benefits will be paid if the insured person is hospitalized for at least five consecutive days and recommended and approved by the attending physician for the evacuation of the insured to his/her home country.
