Waiver Instructions

[All information is subject to change without notice.]

Attention International Students!

The following changes have been made to the health insurance waiver process at The University of Texas at Dallas:

  • You may now submit your waiver by e-application!
  • Expanded eligibility criteria for SHI waivers.
  • Change in required coverage dates for those applying for a SHI waiver.

See below for details!

The Student Health Insurance (SHI) Summer 2008 waiver period begins on April 24, 2008 and ends on Census Day, June 4, 2008. There are NO EXCEPTIONS and NO WAIVERS given after Census Day. This procedure must be followed each semester. It is the student's responsibility to supply sufficient evidence of insurance to the SHI Office within the waiver period. We do ask that students comply with the following information, as this will assist in processing waivers quickly.

The University of Texas System requires F and J international students to maintain approved health insurance coverage while enrolled at U.T. System component institutions. When enrolling in classes, F and J visa holders will automatically be assessed a health insurance fee. The SHI Office (MC 1.310) issues waivers of this fee for students who meet established eligibility criteria to prove that they have comparable coverage to meet the UT System requirement.

Eligibility for a Waiver

A UTD student is entitled to a waiver of the requirement that international students obtain coverage under the System-sponsored student health plan as a condition of enrollment if the student provides documentation that he/she has approved health insurance coverage for the ENTIRE coverage period that meets the minimum requirements.

MANDATORY COVERAGE PERIOD:  The Summer 2008 health insurance mandatory coverage period is 5/23/2008 through 8/20/2008.  In order to be eligible for a Summer 2008 SHI waiver, your alternate insurance MUST be in effect during these dates. Be sure to ask the insurance company of your alternate plan about any possible consequences when there is a break in your coverage!

MINIMUM REQUIREMENTS:  In order to be eligible for a Summer 2008 SHI waiver, your alternate insurance MUST meet or exceed the following requirements:

A. $500 OR LESS DEDUCTIBLE (PER POLICY PER INSURED)

B.  $50,000 OR MORE MAXIMUM BENEFIT PER INJURY OR SICKNESS

C. $7,500 MINIMUM REPATRIATION BENEFIT

D. $10,000 MINIMUM MEDICAL EVACUATION BENEFIT

E.  POLICY PLAN MUST MEET FEDERAL SOLVENCY GUIDELINES

PLEASE NOTE: If you qualify for the waiver and your insurance plan does not include repatriation/evacuation coverage, you will be assessed this fee and enrolled in an approved repatriation/evacuation plan during the waiver process to comply with U. T. System regulations.

The University has the discretion to determine what constitutes sufficient documentation that the student has approved health insurance coverage. Acceptable proof of coverage MUST be written in English.  You are eligible for a waiver if you have been assessed the fee AND fit the criteria of one of the following:

1. Students with UT Select (TA/RA)

UTD employees who have UT Select coverage for the entire coverage period are eligible for an SHI waiver. If Evidence of Insurability (EOI) is required, it must be approved before the end of the waiver period.

Required documentation and Instructions

2. Students with other Insurance

Non-UTD employees who have health insurance coverage for the mandatory coverage period and who can prove that their coverage pays benefits in the United States that meet or exceed the minimum requirements.
