Employees will receive a confirmation email within approximately 24 hours of completing enrollment. Print and keep this email for your records. Evidence of Insurability (EOI) forms, which may be required for life, disability, and long-term care insurance, will be due by Aug. 15, 2014.
In most cases, selections for the 2014-2015 plan year will take effect Sept. 1, 2014, and will be reflected on Oct. 1 paychecks.
If you are an employee who has been hired, become benefits-eligible or experienced a life-change event (i.e. marriage, divorce or birth of a child) between July 1, 2014, and Aug. 31, 2014, contact a benefits specialist within 31 days of the event to enroll. You will not need to go through My UT Benefits.
