  3. 3 letters of recommendation; the request for recommendation form is found at www.utdallas.edu/enroll/graduate .  We request that both the recommendation form and a personal letter be submitted by each reference.

Official transcripts from all the colleges/universities attended.  Each must be in a sealed envelope with the college/university seal stamp.  A copy will be accepted until the original can be sent.
