$200.00 Members • Individuals with current Activity Center Membership
$250.00 Non-members
Scuba Refresher Courses: $80.00
•	Full refund with one week cancellation notice in writing.
•	$20.00 processing fee for cancellations less than one week.
•	Bring your own snorkel, mask, and fins.
•	All other equipment/materials provided.
•	Fill out and return with payment the following Required Medical Forms: – Registration Form – Medical Statement – Physician Form
•	Registration available Monday - Friday in the Recreational Sports Office.
•	Full payment due at registration.
