Pre-Application (Required)

 

To aid the process of determining your eligibility for the SIPID 2008 program, please download and complete the Pre-Application form if you are interested in participating. Please fill out the one-page form electronically or by hand and return by email or fax respectively, to our office. We will review and send you an “Eligibility Decision” within 5 working days of receiving your completed Pre-Application.   

The Pre-Application must be completed before a Formal Application Packet (see below) will be accepted for admission to the SIPID 2008 program at UT Dallas.
