Below are many of our commonly used forms available for you to download. Please mail, fax, or hand deliver completed forms to the Financial Aid Office unless otherwise advised on the form.

Graduate PLUS LBW Form		Instructions on how to accept a graduate PLUS loan on the Loans by Web site.
PLUS LBW Form		Instructions on how to accept a parent PLUS loan on the Loans by Web site.
Parent Loan Borrower Authorization Form		Required form for parent PLUS loan borrowers indicating how funds should be delivered to UT Dallas from the lender.
Standards Of Academic Progress		Information page detailing what criteria is used to determine standards of academic progress as it relates to students receiving financial aid.
Satisfactory Academic Progress Appeal Form		Form used to appeal financial aid suspension.
Financial Aid Hours Adjustment Form		Form used to notify Financial Aid Office that actual hours enrolled are different than originally indicated on FAFSA.
Title IV Aid Refunds and Repayment Requirements		Title IV refunds and repayments insert for award letters. Explains what happens to aid if a student decides to drop classes at UTD.
Consortium Agreement		Form to be completed by academic advisors for off-campus courses. Students must be enrolled in at least 6 credit hours at UT Dallas.
Cancel Aid Form		Form used to request cancellation of aid at school prior to attending UT Dallas.
Certification of Required Preparatory Coursework		Form required for Non-Degree students to receive financial aid. Non-degree students need to be aware that they may only receive aid for 12 consecutive months.
Household Size/Number in College Conflict		Form used to resolve conflicting information reported on the FAFSA and the verification worksheet.
Request for Transcript of Tax Return		Form used to request tax return information.

2008-2009 Special Circumstances: Income Changes: Dependent		Form used to request a review of current financial aid data due to a significant income reduction in a DEPENDENT student’s family.
2008-2009 Special Circumstances: Income Changes: Independent		Form used to request a review of current financial aid data due to a significant income reduction in an INDEPENDENT student’s family.
2008-2009 Special Circumstances: Medical Expenses		Form used to request a review of a family’s current financial aid data due to significantly high medical expenses.
2008-2009 Dependency Override Request Form		Form used to request change in dependency status due to special circumstances.
Verification Worksheets ABC		Form used to correct conflicting information reported on the FAFSA.

Sibling Enrollment Verification		Form used to verify sibling enrollment at other colleges.

2007-2008 Special Circumstances: Income Changes: Independent		Form to possibly adjust aid due to large change in family income for INDEPENDENT students as determined by the FAFSA.
2007-2008 Special Circumstances: Medical Expenses		Form to possibly adjust aid due to high medical expenses.
2007-2008 Dependency Override Request Form		Form required to change your status from dependent on the FAFSA to independent. Additional documentation will be needed as specified on the form.
2007-2008 Student Low Income Letter		Form to use when your income appears to be low for the previous year on the FAFSA and the Financial Aid Office needs to verify why the income was so low.
2007-2008 Parent Low Income Letter		Form to use when your parents income appears to be low for the previous year on the FAFSA and the Financial Aid Office needs to verify why the income was so low.
Non-Degree/Certificate of Required Preparatory Coursework Form		Form required for Non-Degree students to receive financial aid. Non-degree students need to be award that they may only receive aid for 12 consecutive months.
Household Size/Number in College Conflict		Form to help resolve differences in number in household and college between what was put on the FAFSA and the verification worksheet.

Authorization for Title IV Financial Aid Programs		Form that authorizes financial aid to be credited toward any debt incurred during the 2007-2008 academic year.
