Prescription Claims

The Student Insurance Division of the UnitedHealthcare Insurance Company does not require a claim form.  Make a photocopy of the pharmacy prescription receipt listing the drug name, cost, prescribing doctor's name, your name, etc.  Keep this original receipt for your records.  On the photocopy, clearly print the following:

1. Your name and current address
2. Your social security number or the 8 digit number you register for UT Dallas classes with.
3. Date of birth
4. Policy number: 2008-502-1 (for the 2008-2009 academic year)

Mail this copy to:

You should receive a reimbursement check within 45-60 business days from the date you mailed your claim.  You will be reimbursed all but $10 for generic drugs and $15 for brand name drugs.  The prescription limit per academic year is $1,000.  To inquire about claim status, please call 1-800-767-0700.

Medical Claims

Make a photocopy of the original itemized bill.  Keep this original bill for your records.  On the photocopy, clearly print the following: 

1. Your name and current address
2. Your social security number or the 8 digit number you use to register for UT Dallas classes.
3. Date of birth
4. Policy number: 2008-502-1 (for the 2008-2009 academic year)

After you have submitted your claim to the insurance company and it has been processed, you will receive an Explanation of Benefits (EOB) from the UnitedHealthcare Insurance Company.  THIS IS NOT A BILL.  DO NOT PAY IT.  An EOB is an informative statement about your claim (bill) that has been processed. Keep this EOB for your records.  You will then receive a bill from the provider for your remaining balance. The balance due on the bill from your provider should match your balance due listed on the EOB. If it does not match, phone the SHI Office at 972-883-4189 to schedule an appointment.
