Waiver Instructions

The University of Texas System requires F and J international students to maintain approved health insurance coverage while enrolled at U.T. System institutions. Enrolled F and J visa holders will automatically be assessed a health insurance fee. The SHI Office (MC 1.310) issues waivers of this fee for students who meet established eligibility criteria to prove that they have comparable coverage to meet the U.T. System requirement.
Waiver Process
- Find comparable, alternative insurance that meets or exceeds the eligibility requirements listed below.
- Enroll in classes.
- During the waiver period, print and complete a UT Dallas Waiver Form.
- Submit the form to the ISSO with appropriate documentation via email from your UT Dallas email address to issoinsurance@utdallas.edu or in person at the International Student Services office:
- Documents required by students covered by UT Select (TA/RAs)
- Documents required by students with other insurance
The University has the discretion to determine what constitutes sufficient documentation demonstrating approved health insurance coverage. Acceptable proof of coverage MUST be written in English.
Processing Time
You will receive an email from issoinsurance@utdallas.edu within 2 business days to verify that your request has been received and processed. If you do not receive this email, your waiver request has not been processed. Contact the SHI office at 972-883-4189 to inquire about the next steps.
Eligibility
MANDATORY COVERAGE PERIODS:

| Sprng 2009 | 1/12/2009 - 5/25/2009 |
| Summer 2009* | 5/26/2009 - 8/20/2009 |
*Summer coverage is mandatory only if student is enrolled.
REQUIREMENTS FOR WAIVER:
Alternate insurance MUST meet or exceed all of the following:
- Deductible - $500 or less (Per policy, per insured)
- Maximum benefit per injury or sickness - $50,000 or more
- Policy plan must meet Federal Solvency Guidelines
- Repatriation benefit - $7,500 minimum*
- Medical Evacuation benefit - $10,000 minimum*
- Policy coverage dates cover all of mandatory coverage period
- Policy pays benefits while you are in the United States
- You have been assessed the UT Dallas SHI fee
*If your insurance plan does not include repatriation/evacuation coverage, you will be assessed this fee and enrolled in an approved repatriation/evacuation plan.
Waiver Documentation Required for Students with UT Select Health Insurance
Provide a copy of one of the following to verify enrollment in the UT Select Health plan:
- The front and back of your UT Select health insurance card OR
- UT Touch System confirmation document OR
- A TA/RA Benefit Verification form signed and dated by a Human Resources (HR) office representative. This form is available at UTD ISS - Electronic Forms.
If Evidence of Insurability (EOI) is required, it must be approved before the end of the waiver period.
Waiver Documentation Required for Students with Other Insurance
Provide a copy of a letter on the insurance company letterhead OR pages from the insurance policy page(s) that provide the following information:
- Identifies you as a covered individual
- Provides the dates of your coverage
- Clearly indicates that the coverage meets or exceeds the U.T. System minimum requirements, including coverage amounts stated in U.S. dollars.
