Below are many of our commonly used forms available for you to download. Please complete and submit forms according to form instructions. Most forms may be mailed or faxed unless verification of paperwork are required.

Address Change Request pdf   Complete this form to update your address for official transcript processing. Prior students ONLY. Current students use Orion for updating addresses.
Change of Admissions Application Request   website

This form can be used to change a plan on an admissions application for a particular semester. A student can only change an application term forward or back one long semester (fall or spring). The request must be filed by Census Day of the current application term.

To submit a Change of Admissions Application request, log in to Galaxy, then submit an online Change of Application Request web form under the Admission Forms folder.

Diploma Re-order   website Students who graduated from UT Dallas who wish to order additional copies of their diploma may re-order through the UT Dallas Marketplace.
Doctorate Change of Program pdf   For Doctoral students only. Request form for changing program within the same school.
Embassy Letter Request pdf   For students who applied to graduate in the current term only. The purpose of this form is to allow graduating students to invite their family to the commencement ceremonies at the end of the semester.
Enrollment Certification -
National Student Clearinghouse

Current students fall 2011 and after, may connect with the National Student Clearinghouse using the 10-digit UTD-ID and obtain copies of documents confirming enrollment at UT Dallas for personal records.

Students needing enrollment certifications for auto insurance, scholarships, health insurance and with GPA information may not use the National Student Clearinghouse service. Please follow the Enrollment Certification - Orion Enrollment Verification option.

Examine Student Record Request pdf   Students complete this form and return to the Office of the Registrar. Information may not be available on demand; therefore, an appointment may be scheduled for the examination of your records.
FERPA Packet pdf   Request packet includes: Request for Confidentiality, Parents Certification of Dependency, and Third Party Release Request
Graduation Verification Letter pdf   This form is for requesting a letter that will state that the student has earned a degree from the University of Texas at Dallas.
InAbsentia Forms - Undergraduate and Graduate pdf   Applying to graduate in absentia allows a candidate to apply to graduate without formally enrolling in courses for that semester. The .pdf contains both the undergraduate and graduate in absentia forms. Please complete the form appropriate to your career.
Intent to Participate Form pdf website This form is required for all summer graduation applicants.

Meningococcal Meningitis Vaccination Requirement Form pdf website

Per new State legislation beginning January 1, 2012, all entering Texas college students must receive a vaccination or booster (if the vaccination is five years old) against bacterial meningitis before enrollment.

Complete the form and supply the necessary documents to the Office of Admission and Enrollment Services in the Student Services Building no later than 10 working days before the semester begins. Entering students will be unable to register until the paperwork is received and reviewed. This includes registration at orientation.

The fall 2014 deadline is August 15, 2014
The spring 2014 deadline was January 3, 2014
The summer 2014 deadline is May 18, 2014

Students may fax or mail the form and supporting documentation.

Petition for Graduate Deferment of Admission   website

A statement of reason and any supporting materials the applicant believes to be relevant and wants the Academic Program to consider should be submitted with this form.

To submit a Petition for Graduate Deferment  of Admission, log in to Galaxy, then submit an online Graduate Deferment  of Admission web form under the Admission Forms folder.

Petition for Undergraduate Deferment of Admission website

The deferment is only valid for the semester of the student's original application. Admission can only be deferred for one semester unless the student is a member of the Transfer Comet Connection program. Deferment request must be filed by Census Day of the original application term.

Petition for Undergraduate Student to Take Graduate Course   website For undergraduate students only. Undergraduate students wishing to enroll in graduate courses must complete a petition form with their advisor.
Petition to Use Military Credit pdf   Complete this form to petition awarding transfer credit for military training. Please include the appropriate documentation with the form and submit to the Office of the Registrar.
Petition to Use National Test Scores as Transfer Credit pdf website For undergraduate students only. Undergraduates students complete this form to receive transfer credit from appropriate test credit. Please review the undergraduate dean's website for details regarding test credit that applies.

Transcript Certification Letter pdf  

This form will certify; Test Credit and UTD coursework that does not appear on the Official Transcript.

Please fill out the PDF form and turn in to the Office of the Registrar in the Student Services Building for processing.

If a certification letter needs to be attached to an official transcript, please request and receive the letter BEFORE ordering an official transcript for medical/ law/ dental/ etc application services.
