Students needing enrollment certifications for auto insurance, scholarships and health insurance may not use the online service. Please fill out the provided form.

Name Change Request

Texas Tuition Rebate

  For Undergraduates Only. Application to receive a $1,000 tuition rebate for qualified undergraduate students. The application is to be filled out in a student's graduating term.
Official Transcript Request

Request form for an Official UTD transcript. Requests can be mailed only to the requestors address on file or to a university address. Please check your address through our student information systems before requesting one sent to your personal address.

If you have a hold on your record, your request will not be processed. Once your hold is removed, you will need to resubmit your request.

Undergraduate Change of Major or
Addition/Deletion of Degree, Major, or Minor
