HR Forum Scheduled for Dec. 1
The Office of Human Resources will host its next HR Forum from 10-11:30 a.m., on Wednesday, Dec. 1, as a Microsoft Teams live event.

Online Consent To Receive Form 1095-C for Calendar Year 2021
In accordance with the Affordable Care Act (ACA) provisions, UT Dallas will have the 2021 Form 1095-C available on or before Jan. 31, 2022. The Form 1095-C includes information about the health insurance coverage offered to employees, retirees and their dependents. You will receive the Form 1095-C if you were enrolled in the UT Medical Plan or if you worked an average of 30 or more hours per week or were considered a full-time employee in 2021.
Graduate student employees who are covered under the UT Student Health Insurance Plan will receive Form1095-B directly from Blue Cross Blue Shield.
If you provide consent, you can access your Form 1095-C online through Galaxy. If you provided consent in prior years, you do not have to do so again. The deadline to provide online consent is Jan. 17, 2022. If you do not provide the online consent, you will receive the form in the mail on or before the IRS deadline. The forms are expected to be available online by Jan. 31, 2022.
If you need to provide online consent, log in to Galaxy with your UT Dallas NetID and password. Then, go to Self Service, select ACA Form 1095-C and follow the on-screen instructions. Written instructions and FAQs are posted on the HR website.

Volunteers Needed for Commencement Ceremonies
Staff members interested in volunteering at the Fall 2021 Commencement Ceremonies are invited to sign up by Monday, Dec. 6. Commencement roles include greeters, ushers and shuttle guides. Available shifts include:

Year-End Giving to UT Dallas
The Office of Development and Alumni Relations will accept gifts made to the University for 2021 income tax purposes. During the campus closure (Dec. 27-31), call the department’s main phone number at 972-883-2295 for information regarding on-call personnel.
Please see the instructions below to learn about proper handling for year-end gifts made by various payment types.
By Check
In order to process contributions made via checks in 2021, we must be able to provide proof that the gift was postmarked by Dec. 31. For any checks received through the month of January 2022, please make sure that the mailing envelopes are attached to the checks before delivering to the development office. According to IRS rules, the postmark is the deciding factor and must be dated by Dec. 31, regardless of what date is listed on the check.
By Credit Card
Credit card gifts are counted in the tax year charged to the donor account. For gifts via credit card, please direct individuals to give online. Gifts made online by Dec. 31 will be processed automatically before midnight and thereby count as 2021 gifts.
By Wire Transfer or EFT/ACH
Gifts via wire transfer or EFT/ACH are counted in the tax year they are credited to the UT Dallas account. Email [email protected] for instructions.
By Stock Transfer
All stock transactions benefiting UT Dallas are handled through the UT System Office of Gift Administration, Compliance and Advancement Services. It is best if the donor’s broker/financial consultant speaks directly with one of our UT System contacts:
Please advise any donor wishing to make a stock gift to connect with a UT System representative as soon as possible. Requests made after Dec. 17 may not be processed until after the new year.
IRA Charitable Rollovers/Qualified Charitable Distributions
Donors should check with the plan administrators to determine the necessary time to make a direct distribution to UT Dallas.
Complex Assets (Real Property, Partnership Assets, Etc.)
These types of gifts require due diligence and official acceptance by UT System. Please contact us as soon as possible to determine if UT Dallas can accept the contribution by year-end. For more information, please call 972-883-2295.

Callier Center Event
Callier FLASH Seminar: Exploring the Connection Between Brain Health and Mental Health, noon-1 p.m., Friday, Dec. 10 — Sarah Laane MS’19, a doctoral student in speech, language and hearing sciences at UT Dallas, is working on identifying how frontal lobe engaging strategies improve brain health and cognitive performance in the general population and vulnerable populations including justice-involved youth, those with mental illness history or symptomology, and those from underserved communities. Center for BrainHealth Events
Intersections of the Brain and Technology, noon, Friday, Dec. 3 — Dr. Lin Lin’s research looks into intersections of mind, brain, technology and learning. She has published in areas including creativity, virtual reality, media multitasking, multimedia design, CSCL, critical thinking, computational thinking, and learning in virtual spaces. She serves as director for the Texas Center for Educational Technology (TCET), and development editor-in-chief of the journal Educational Technology Research and Development.
The Confident Brain: The Science of Self-Belief, Sponsored by Bank of Texas, 7 p.m., Thursday, Dec. 9 — Confidence lies at the core of what makes things happen. Dr. Ian Robertson, co-leader of the BrainHealth Project, is a leading researcher on how we can harness the mind’s attention systems to maximize the brain’s capacity to reshape itself and to give every person a sense of control over their own emotions and cognitive function.

Reminders
Sign Up To Receive Your W-2 Electronically
The UT Dallas Payroll office will issue W-2s by Monday, Jan. 31 to current and former employees who had wages in calendar year 2021. If you would like to receive your W-2 electronically instead of having to wait for it to be printed and mailed, please follow the steps below before Monday, Jan.10. Your election remains in effect for all future calendar years, it does not need to be renewed each year.
- Log in to Galaxy.
- Click “NavBar: Navigator” in the upper right corner, under Main Menu select “Gemini HCM.”
- Under “Self Service,” select “Payroll and Compensation.”
- Select “W-2/W-2c Consent.”
- Check the box that says “Check here to indicate your consent to receive electronic W-2 and W-2c forms.”
- Click “Submit.”
You will receive a confirmation email at your UTD email address when the change is made.
Benefits of the electronic W-2 include earlier receipt of the form and the ability to access and reprint the form at your convenience. If you will be accessing your W-2 from an off-campus computer, you will be required to sign up for NetID Plus. Former employees can still access Galaxy for up to three years from their last day of work to access paystubs and W-2 forms.

Sign Up for Mobile Mammography
The UT Southwestern mobile mammography unit will visit the loading zone north of the Activity Center from 8 a.m.-3 p.m., Friday, Dec. 10. 3D scans will be available, and most insurance plans will be accepted. Contribute Pandemic Memories to UTD Time Capsule
The Eugene McDermott Library and University Archives are launching the Comets Endure Digital Time Capsule, a digital memory project to honor students, faculty and staff who have endured through the COVID-19 pandemic.
We have collectively experienced a dramatic moment of UTD’s history unlike anything before or possibly yet to come. This digital collection will preserve stories and memories of this time so future UTD classes will remember. The time capsule will be opened in 2041.
If you would like to contribute your pandemic memories, please complete the submission form. Submissions will be accepted through the end of the 2021-22 academic year. Comets United. Comets Endure!
